About Us
Monument Rehabilitation and Care Center is a short-term, long-term skilled nursing and assisted living facility. In the skilled nursing facility, all residents receive 24-hour routine and emergency medical care and are in constant interaction with our caring, skilled nursing department who provide continuous physical and emotional support while promoting mobility and flexibility. Our attending physicians, nurses, therapists, and social workers work together to provide every resident with an array of services customized for their needs, including restorative care, hospice and palliative care, physical therapy, occupational therapy as well as speech therapy, and dietician services.
At Monument we have a dedicated unit to work with residents suffering from Dementia and Alzheimer’s. Our clinical team works hard to ensure each resident is constantly mentally and emotionally stimulated and assisted with living their best life in a safe environment.
Monument Rehabilitation and Care Center’s assisted living offers assistance with daily living activities in a community like setting. Residents are given supportive care in: medication administration, routine meal provision, and assistance with bathing and laundry as needed. Service are provided in private apartment settings. Residents are encouraged to maintain social well-being with a wide variety of activities.
