About Us

Brookestone Home Health and Hospice is a Medicare/Medicaid Certified Home Health agency located in the Panhandle of Nebraska in Gering Nebraska. We are licensed in 19 counties in the panhandle of Nebraska and one county in Wyoming for Hospice. We are currently providing services to 12 of those counties with the hope of expanding into the other 7 in the next year! We are currently offering Home Health skilled services in the form of Registered Nursing, Licensed Practical Nursing, Physical Therapy, Physical Therapy Assistant, Occupational Therapy, Home Health Aide, and Social Work services that all come to you in the home. Hospice services offer Registered Nurse, Licensed Practical Nurse, Home Health Aide, Social Work, and Spiritual Care. We work with all the local and non-local physicians to provide you with the best care possible in the environment where you feel the most comfortable, your own home! Home Health will come into your home and perform an evaluation with a physician’s order based on a skilled need and look at managing a specific medical condition or recovering from an illness or injury, requiring skilled services like wound care, medication management, or therapy sessions, as prescribed by a doctor.

Hospice care is a type of end-of-life care that focuses on comfort and quality of life for people with serious illnesses. It's designed for patients who are terminally ill or have a life expectancy of six months or less.
